Scary story Pete, glad nothing worse happened. Just a few tech tips I learned over the years to help others avoid your scary situation...
1. Never finger tighten anything, always leave the bolts or nuts loose by a couple of threads so you can se them.
2. I always take a paint pen and mark the bolts I have tightened just so I don't have to remember if I tightened them. If they are painted they are tight.
3. Saftey wire doesn't hurt either.
